Schedule an Annual Wellness Visit 

This appointment allows your doctor to paint a picture of your health. Bring the following information to share:

  1. Your up-to-date health history. You’ll likely be asked to fill out or update a new patient form. 
  2. Symptoms or concerns. Even if it’s small, bring it up. 
  3. A list of your current medications. This includes supplements and over-the-counter medications, including when you take them and how much. Better yet, bring the bottles so the doctor can review the labels directly. 

Don’t forget your medications 

Did you know that 40 percent of patients with chronic conditions don’t take their medications, often because they simply forget? Remembering to take your medications exactly as prescribed keeps you strong and healthy. 

Try these three simple tips:

  1. Use a pill box. It can help you organize daily doses.
  2. Set a reminder on your phone. When you hear the ping, it’s time to take your meds.
  3. Link meds to a daily activity. Connect doses to a routine like breakfast or bedtime. 

Schedule your health tests 

While everyone’s personal care plan will be a little different, ask your doctor about these screenings and vaccinations often recommended for those over 65:

  • Colon health (every 1 to 10 years from ages 50 to 75, depending on the type of test)
  • Osteoporosis (once at age 65 or older, or earlier if you have a history of bone density loss)
  • Immunizations for the flu, pneumonia, and hepatitis B (as recommended by your doctor)
